Assessment of Equity in Beliefs and Practices of Teaching Mathematics to African American Students
This assessment is designed to assess the mindsets of mathematics teachers at the middle and high school levels in teaching mathematics to African American students. The purpose of the survey is to provide an indication of (a) teachers’ mindsets that may be productive or unproductive in supporting the mathematical learning of African American students; (b) teachers’ awareness of and attribution for success and challenges in African American students’ learning of mathematics; and (c) teachers’ feelings of self-efficacy and/or support in enacting equitable instruction.

Beck Youth Inventories - Second Edition (BYI-2)
Five inventories each contain 20 questions about thoughts, feelings, and behaviors associated with emotional and social impairment in youth. Children and adolescents describe how frequently the statement has been true for them during the past two weeks, including today.
Depression Inventory: In line with the depression criteria of the Diagnostic and Statistical Manual of Mental Health Disorders Fourth Edition (DSM-IV), this inventory allows for early identification of symptoms of depression. It includes items related to a child's or adolescent’s negative thoughts about self, life and the future, feelings of sadness and guilt, and sleep disturbance.
Anxiety Inventory: Reflects children's and adolescents’ specific worries about school performance, the future, negative reactions of others, fears including loss of control, and physiological symptoms associated with anxiety.
Anger Inventory: Evaluates a child's or adolescent’s thoughts of being treated unfairly by others, feelings of anger and hatred.
Disruptive Behavior Inventory: Identifies thoughts and behaviors associated with conduct disorder and oppositional-defiant behavior.
Self-Concept Inventory: Taps cognitions of competence, potency, and positive self-worth.

Behavior Assessment System for Children, 3rd Edition (BASC-3)
The Behavior Assessment System for Children (BASC), which is sold by Pearson, aims to measure adaptive behaviors, problem behaviors, and individual thoughts and feelings. It contains multiple evaluation forms: Teacher Rating Scales (TRS) and the Student Observation System (SOS) to be completed by a teacher; Parent Rating Scales (PRS), the Parenting Relationship Questionnaire (PRQ), and the Structured Developmental History (SDH) to be completed by a parent, and the Self-Report of Personality (SRP) to be completed by the student. Each form can be completed separately or in combination.

Behavior Rating Inventory of Executive Function (BRIEF)
The BRIEF was developed in 2000 and assesses a student's executive functioning in the home and school. It includes parent response forms and teacher response forms. Designed to assess the abilities of a broad range of children and adolescents, the BRIEF is useful when working with children who have learning disabilities and attention disorders, traumatic brain injuries, lead exposure, pervasive developmental disorders, depression, and other developmental, neurological, psychiatric, and medical conditions. The BRIEF has eight clinical scales (Inhibit, Shift, Emotional Control, Initiate, Working Memory, Plan/Organize, Organization of Materials, Monitor) and is helpful in indicating attention disorders. The BRIEF has been adapted into 60 languages and for use from age 2-90. Recently, the BRIEF2 was created for ages 5-18. The BRIEF2 is shorter, but has no additional clinical items, allowing for consistency of data collection between the BRIEF and BRIEF2, and results can be translated from the BRIEF to the BRIEF2 to take advantage of new normative data. The BRIEF2 also has increased sensitivity for detecting ADHD and ASD.

Mathematics Teaching Efficacy Beliefs Instrument (MTEBI)
The Mathematics Teaching Efficacy Beliefs Instrument (MTEBI) is a domain-specific modification of the Science Teaching Efficacy Belief Instrument (STEBI). The MTEBI measures pre-service teachers’ beliefs regarding mathematical instruction, students' ability to learn mathematics and teachers’ sense of efficacy in teaching mathematics.

National Survey of Science and Mathematics Education (NSSME)
The National Survey of Science and Mathematics Education (NSSME) is a periodically administered national survey that collects information from teachers and administrators about STEM offerings in schools. The teacher surveys cover a broad range of topics, including demographic information, experience, pedagogical approaches, professional development, curriculum use, and collaboration. The survey also collects information about teachers' perceptions of how schools are encouraging and supporting student participation in STEM.

Preservice Teacher Feedback Survey
The Preservice Teacher Feedback Survey measures beliefs about classroom feedback practices and confidence in giving feedback to students. The measure is designed for use by preservice teachers to assess the extent to which their beliefs and confidence in giving feedback align with research on quality feedback. The measure has been used as a pre- and post-test to assess the effects of initial teacher education courses on assessment.

Science Teaching Efficacy Belief Instrument (STEBI)
The Science Teaching Efficacy Belief Instrument (STEBI) can be used to measure science teachers’ self-efficacy and outcome expectancy across all levels of teaching. The STEBI-A is the version recommended for use with in-service teachers; the STEBI-B is the version recommended for use with pre-service teachers.

Teacher Sense of Efficacy Scale (TSES)
The Teachers' Sense of Efficacy Scale (TSES) measures teachers' evaluations of how likely they are to be successful in teaching. TSES conceptualizes teaching as a complex activity and teacher efficacy as a multi-faceted construct representing at least three distinct factors: Efficacy for Classroom Management, Efficacy to promote Student Engagement, and Efficacy in using Instructional Strategies. It is designed for and has been used by researchers and school leaders to measure teacher self-efficacy at a particular point in time, as well as before and after participating in professional development programs. There is a short- and long-form version of the scale.

Technological Pedagogical Content Knowledge (TPACK)
The Technological Pedagogical Content Knowledge (TPACK) instrument is a self-efficacy survey that seeks to monitor and assess teachers' development of TPACK and seven interrelated knowledge domains. The measure is primarily aimed towards pre-service secondary mathematics teachers and can be used to longitudinally study program effectiveness at TPACK development, and to conduct large-scale multi-institutional research studies (Zelkowski, et al., 2013).
